The Permit Application form is displayed when creating a new permit or renewing an existing permit.  All fields are required on the form.

Step One To select an exam location other than the default, click the dropdown arrow. Enter the date of birth in mm/dd/yyyy format. The permit type options will vary based on the action you are preforming. View the medical definitions PDF for more information. If there is more than one valid medical condition for your patient, you need to only select one for the permit application. Select save once the application is complete. This will not submit the permit application to the DMV. Further action is needed.
